(B) Community Health Nursing Diagnosis Statement
The community health nursing diagnosis statement is quite broad in every sense of the word because it entails establishing the problem symptoms, then the actual diagnosis and then followed by the etiological factors. In my case, I wish to identify the effect of high sugar intake in the community as a health hazard. The identification will be in terms of their lifestyle habits such as buyingreadymade sweets and taking dense breakfast. The other assessment that will be done in this exercise is assessing the playing fields and the gym equipment’s in order to promote self-care and eliminate lifestyle diseases.
(B1) Health Inequity/Disparity
Social economic inequalities such as income levels, occupation and education have been distinguished to create inequality gap in the community and this is linked to obesity. The economic position and a person’s personal social position in relation to others has been linked to have a direct effect on lifestyle and obesity. Most importantly Self-care promotes individual health, lack of exercise was established to be a major cause of injury especially to overweight patients.
Self-care is promoted through deliberate efforts to the lifestyle of an individual therefore a promotion talk about the recommended foods was given. Specific foods that provide energy to the body are provided and more emphasis was done on vegetables.
Obesity in the community has affected individuals from all ages young and old which puts them at a risk of lifestyle diseases such as hypertension and diabetes and even cancer. Most people in the community who were obese were suffering from either of these lifestyle diseases therefore there is need to check on exercise and diet. Evaluation on nutrition indicated that obesity would be prevented through weight loss and health improvement strategies. Also during patient interviews most questions from the patients pointed to management and treatment therefore poor lifestyle choices led to health lifestyle diseases.
This information was relayed by a physical assistant and patients in the training camp. Physical exercises such as boxing and other functional exercises among various age groups together with rich diet were linked to healthy living with minimal injuries.
(B1a) Primary community and Prevention Resources
Obesity as a great challenge in the community will be overcome through a number of resources, one is the healthy hospitals in the neighborhood. Employees and hospital management educating individuals on benefits of healthy lifestyle will impact the society greatly. Early education facilities such as playing fields and gym will ensure children at an early age become physically active and avoid obesity. Community resources include the Mosaic central farm market that will be the venue will start the journey of making the plate great among residents. Lyn will create social media awareness in this market through messages such as overeating is unhelpful to the body and need to develop emotional trigger that enables people to be mindful of what they take. Through the sunrise senior living center in Alexandria residents will be provided with educative resources on self-care and strong emphasis on cutting down on raw sugar.
Plat fitness will be another prevention resource where personal training programs will be initiated through social media and provide knowledgeable resource on the exercise programs. Privatedoctors and physician assistantwill have sessions with patients and assist them on the journey of management of the lifestyle diseases. Ballston children center will be another primary community resource that will offer free classes to children and involve them in sports from an early age such as rowing.
The Virginia healthcare services will assess the community resources and advise them on different ways to maintain better health. A fitness class conducted at Dulles Family medicine will provide community resources assessment the class will focus on stretching muscles and small movements that reduces cholesteryl levels in the body therefore reducing obese members in the community.
(B1b) Underlying Causes
One potential contributing factor to the health concern in the community is nutrition as elaborated by the mother with special needs. This woman depends on special needs for her child who is in critical nutritional wants, she notes that her child need nutritional value foods so as to avert lifestyle diseases. The information provided in the training is helpful and allows low card options. Lack of enough playing fields and gym facilities that charge high fees are some of the factors that hinder individuals to lead a healthy lifestyle in the community. There are very few trainers on healthy lifestyle therefore a large population in the community is ignorant on healthy lifestyle through exercises and healthy diet.
A cancer specialist notes that nutrition and diet has an impact on cancer and therefore some of the modifiable risk factors can be eliminated through change in lifestyle. Through assessing the community resources healthy activities and programs were designed to allow even the low income families to enjoy healthy foods and exercise as advised by the doctors.
(B2) Evidence-Based Practice
Some preventable chronic diseases such as type two diabetes mellitus, obesity, hypertension and cardiovascular diseases can be prevented through self-care interventions such as exercise and better diet (Karlin& Cross,2014).It is a fact that most patients understand the reasoning behind healthy lifestyles as advocated by doctors and the repercussions of not maintaining a healthy lifestyle but the challenge is self-discipline.
Despite this knowledge of lifestyle, there is evidence that many patients lack proper behavioral skills that they can follow on a daily basis that would lead them to the healthy lifestyle. Healthy lifestyle modification is usually done when it is too late, when lifestyle diseases have already set in, through theses interventions are successful there is need to have them earlier on when diseases have not set in. Karlin and Cross(2014), has elucidated that appropriate interventions such as exercise training, nutritional counseling, stress management have been proven to improve the patient outcome in health care almost immediately.
(B2a) Identification of Data
In California the research indicates that in addition to preventable interventions to health care, employers offer more than eighty-six percent of health promotion activities which include, healthy food options,on-site vaccinations and employee assistance programs.
When wellness is compared at a national and local level to gauge its overall contribution to health care there are clinically meaningful improvements in smoking behavior, exercise frequency, and weight control but not in cholesterol control.
(C1)Social Media Campaign Objective
Makary and Daniel (2016), indicated that social media marketing had become an integral part of marketing in many companies. Therefore one of the social media campaign objectives will increase exposure to health care through reaching as many young people as possible. Another goal that I wish to achieve through social media is increasing the number of engagements through comments, likes, shares, retweets, and favorites. Through social media, I will also be able to develop possible business leads and penetrate new markets that were not reached before. Social media platform has a possibility of establishing a loyal fan base that improves sales and possibly grows business partnerships.
(C2)Social Marketing Interventions
Two critical marketing interventions in my social marketing intervention will be learning and listening. Patients will make their complaints through the social media platforms so as to note the areas of improvement. When a query is put forward on the Facebook page the response will benefit the whole community and not only the concerned patient. Also in the Facebook or whatsapp group individuals across the community will share stories that will inspire others and this will be a remarkable journey to overcoming obesity.
Social media outlets such as a Facebook page has potential to reach a high population, at the same time it maintains privacy as patients will inbox their issues which will be directed to a specific doctor. Depending on the issues raised, patients will be grouped together if they hasve similar challenges to form a class for instance a fitness class for the obese. Cancer patients will also be grouped together and for each group a team leader will be appointed. In order to maintain cohesion they can form a WhatsApp group that will share information and materials that are relevant to the group.
(C3) Social Media Platform
Through Facebook and WhatsApp as my main social media outlets since they tend to appeal to a large population. For a WhatsApp group community members of a specific group with similar challenges will share relevant materials in the group and can hold meetings regarding their strategy online. The exercise timetable can be updated regularly on WhatsApp groups and ensure that every member is participating in the program.
ForFacebook the members of a similar group will be able to form a group and share information to both group members and nonmembers therefore they will have a bigger audience. These two social media platform allows members to group themselves depending on their needs and age, for instance the CrossFit for children will provide information that is relevant to their nutrition and diet.
(C3A) Benefits of Social Media Platform
The two social media platforms, Facebook and WhatsApp will support preventive healthcare because patients will have somewhere to belong. They will group themselves according to their challenges and share their predicaments through this sharing they start the journey of healing. Social media especially Facebook has a lot of material available for the patients to utilize on healthy living. Healthy living will have to be incorporated into the daily living habits, such as social media, WhatsApp and Facebook. Therefore patients will share preventive healthcare information such as videos on the progress made in weight loss which serves as a motivating factor.
(C4) Benefits to target population
Facebook and WhatsApp will mainly benefit the community through clustering of groups according to the challenges. For instance individuals from age 25-35 years will a WhatsApp group and through this platform they will provide educative resources on self-care since they are at risk of becoming obese since most are not physically active. The avoidance of sugar in the diet and programs that indulge in small sweetness. The WhatsApp group can be named as Kick the sugar habit which resonates with the challenge they want to overcome. Materials regarding healthy diet foods that have low sugar will be beneficial to this group and will be shared in plenty. The cost of sharing information through social media platforms is quite low and instant therefore appealing to the community.
Through Facebook platform members that are obese can be grouped together and share relevant information on exercise and basic training movement. One benefit with Facebook will be members can share videos of exercises as well as the exercise improves what part of the body.
(D) Best Practices for Social Media
Web-based social networking has turned into a shorthand assignment for individual to-individual data shared utilizing a wide assortment of on the web and computerized stages, for example, Facebook, Twitter, Pinterest, Tumblr and LinkedIn (Karlin& Cross,2014). Most began as work area applications, yet all have encountered emotional development in their change to versatile stages.
Web-based social networking applications initially were intended to enable people to make self-characterized systems of companions or similarly invested outsiders and offer data, stories, connections, and pictures. As web-based social networking turned out to be more common in the late 2000s, business intrigue detonated. Organizations saw online networking as another channel to convey their messages to their groups of onlookers.
Organizations are not the sole wellspring of put stock in data via web-based networking media. Indeed, a significant portion of the data that individuals depend on originates from companions, partners and now and again associations that they have self-chosen to join their informal organizations. (Boulware, et.al 2016), demonstrates that 41% of clients say online networking would impact their decision of a specialist or doctor’s facility—and 9 out of 10 18-to 24-year olds would trust therapeutic data others shared via web-based networking media. It’s imperative to recollect that those 18-to 24-year olds are tomorrow’s patients, individuals, and representatives. Plainly, online networking is a primary specialized device, yet one that requires the other dynamic and diverse range of abilities than conventional media.

Through the social media platform one of the assessment of the effectiveness of the campaign will be increase in membership. Therefore after the six months a review will be conducted to measure new members who have joined the group after six months are over. The information regarding healthy living shared on Facebook was it helpful? This will be evaluated by the feedback from the comments or likes associated with the comment.
The other evaluation criteria is that the whole process was supposed to be self-sustaining in that individuals were to take up the challenge and own it as their process therefore checking if they youths have taken up personal training will be essential.
(E5) Cost of Implementing the Campaign
The cost of this community health clinical time log for students in the unregulated states is estimated to be $500.
(F)Reflection on Social Media Marketing
Community nurse has a duty and obligation of ensuring the best possible care to their patients, social media has provided a direct platform that allows a nurse to interact freely with a patient at any given time. Social media influence has grown tremendously in the society in the recent past and therefore community nurse will find alternative expertise to provide care online and instant without patients visiting the hospital setting. Social media has enabled community nurse to connect and offer support to patients that would have been restricted by geographical location. When a community nurse want to receive information from a patient they can easily email it and ensure confidence therefore integrating social media and occupational practice.
(F1)Reflection on Future Nursing Practice
Social media has eased the way of life, for a community nurse whatsapp groups of the different age groups will be essential in keeping the individuals informed of the workout areas for instance on a daily basis.Holding a meeting will no longer require physical presence it can be done through a whatsapp group which saves time and it’s more efficient.
Nursing profession has been greatly impacted by social media because social media has simplified the whole care process. Patients will no longer have to travel long distances in search of medical care this will be available online through social media platforms. In the future clinicians will have to change from the current setting of face to face central resource to a setting of sharing health information online. This will have a greater scope of patients than it is currently is and there will be proper diagnosis among patients due to a lot information available online. Health information and communication will educate as many patients as possible.
